ENCLOSURES/CHECK LIST
i. Attested photocopy of Pass Certificate and Mark Sheet of Class-X Board Examination.
ii. Photocopy of Admit Card provided by XII/+2 Board (CBSE/ISC/CHSE) for those who have
appeared class-XII/+2 /CHSE Examination.
iii. Three self attested passport size photographs.
iv. One self addressed envelopes with affixed stamp of Rs.5/-.
NOTE:
1. The candidates are required to collect admit card at the time of submission of the form from
the office.
2. Students who have downloaded the admission form from the website are required to send Demand
Draft of Rs. 400/-(Rupees Four hundred only) of any nationalized bank drawn in favour of "DAV
School of Business Management," Bhubaneswar.
3. Successful candidates will be sent intimation for provisional admission. Candidates can also see their
results from our website www.dsbm8.org and College Notice Board (6-05-2011 at 10:30 AM).
